San Luis Valley Health (SLV Health) is the largest healthcare provider in the San Luis Valley, located in south central Colorado. The organization runs a state-of-the-art regional medical center, a county hospital and five clinics scattered throughout the valley. SLV Health also manages several outreach programs to improve the health of Valley residents.
Our Work for this Client
After working together since 2010 on a successful grant-funded community outreach project called “Get Healthy SLV”, SLV Health asked Upleaf to create an institutional website for the organization in 2013.
The new responsive-design website was optimized for mobile and tablet traffic, and includes interactive features to help visitors find the services and physicians they're looking for. It also includes a Patient Center with a library of health information, a dedicated "Your Health" section with health tips and programs, and a health quiz to help visitors identify ways they can improve their overall health.
In an effort to reach the region's sizeable Spanish-speaking population, Upleaf also designed and created a Spanish section for the website.
Constituent Relationship Management Systems
In 2011, Upleaf customized Salesforce to help SLV Health manage the "Get Healthy SLV" project.
At the time, the organization offered a searchable directory of health, nutrition and wellness providers across the Valley. SLV Health used Salesforce to house all of the provider information, send e-newsletters with health tips to the Valley community, and report on a community-wide "Coming Alive" wellness program.
In 2013, Upleaf further customized the CRM system to help SLV Health manage new grant-funded Diabetes and Tobacco Cessation projects. The system streamlines client intake, helps navigators record follow-up interactions and track outcomes over time. Salesforce simplifies the donor reporting process and allows grant managers to keep tabs on progress using simplified, visual dashboards. Project staff can also use the system to send emails and surveys to clients.
The system was also adapted to allow SLV Health's marketing team to manage their email communications with SLV Health employees and the general community.
Digital Social Marketing
As part of the "Get Healthy SLV" project, Upleaf helped SLV Health establish their presence on Facebook, Twitter and YouTube in 2011. Starting from a community of zero supporters, we branded the platforms to match the project website, and created a series of print materials to raise awareness of the project and social media platforms within the San Luis Valley Health hospitals and clinics.

We then worked closely with the project manager to provide extensive best practices training.
Because managing relationships with local partners and community members was of utmost importance, Upleaf set up branded templates on Vertical Response and shored up the project's email marketing strategy. Upleaf also helped the project secure a Google Grant and set up Adwords campaigns which effectively doubled site traffic during the last year of the project.

More than 8,500 people in the Valley alone were reached through the project, representing over 25% of the adult population.
Coming Alive App
In 2013 Upleaf built a mobile app to manage a "Coming Alive" contest among local employers, which allowed employees to log their exercise, fruit, vegetable and water intake every day. Upleaf also created campaign images and a project brand for the Coming Alive project to help raise aware about the contest across through their website, social media presence and email.

After the Get Healthy SLV project period ended in October of 2013, Upleaf rebranded all social media platforms and email templates as San Luis Valley Health, integrated them with the new website- and then passed the baton for online community engagement to the organization's marketing department. The project was chalked up as a win--having reached tens of thousands of people and meeting the project goals for educating the community.
Monitoring & Optimization
Upleaf met with the Get Healthy SLV project team each quarter to review project progress and discuss ways to improve outcomes. We prepared a quarterly report that aggregated results across social media, email and website platforms, and evaluated levels of engagement and progress toward project goals.
Upleaf also programmed key conversions into Google Analytics, and automated monthly reports with key indicators to be sent to program managers by email.
Along with the monthly reports, we designed reports to measure specific initiatives like Coming Alive, and crafted an infographic to report results to the community. When the project wrapped, we produced a report with recommendations regarding email marketing lessons learned, and synthesized a final presentation on project results, particularly addressing how the equity built through the project could benefit San Luis Valley Health overall.
